Discontinuation of Attendance Online Form
To facilitate the effective processing of discontinuation of attendance online forms for the Deree Sports Academy, the Parent/Guardian 1, submitting the application is kindly requested to complete all fields of the online form, after consultation with Parent/Guardian 2.

Upon submission of this form, an e-mail will be sent to both email accounts of Parents/Guardians, as they appear in the Deree Sports Academy registration form.
The online form must be completed and submitted by the 25th of the month preceding the month of discontinuation, so as not to be charged for the following month or months.

Otherwise, the monthly fee will be charged as usual, and the discontinuation will be effective from the following month.
